What is Barrier Failure Analysis?

Barrier Failure Analysis (BFA) is a systematic approach to incident analysis. It examines the sequence of events, the state of barriers, and the causal path that led to the incident. The method is based on the Swiss Cheese Model and Human Behaviour theory, and is used to identify weaknesses in technical systems, human actions, and organisational processes. The aim is to provide clear insight into why an incident occurred and how similar events can be prevented in the future.

The course

The course is designed to build your incident analysis skills step by step in order to apply Barrier Failure Analysis (BFA) for your incident investigations.
You will practise identifying events, analysing barriers, and constructing the causal path. By tracing underlying causes, you will be able to move beyond symptoms to uncover meaningful improvements.

The e-learning consists of a theoretical part with exercises and an exam. After completing the theory and exam you can claim your certificate of completion.
